HEALTH COVERAGE TAX DOCUMENTS

In compliance with the Paperwork Burden Reduction Act (PBRA), Revitalize Capital furnishes Forms 1095-B and 1095-C electronically. Employees may access an electronic copy through Paycor under Pay Stubs & Tax Documents, alongside their Form W-2. A paper copy is also available upon request.

You may request a copy of your 2025 Form 1095-B or Form 1095-C by contacting us using one of the methods below.

Upon receipt of your request, we will provide the requested form within 30 days, or by January 31 of the year following the reporting year, whichever is later.

This notice applies to the 2025 calendar year and will remain available on this website through October 15, 2026.
